Departure of children under the age of 16 outside of Ukraine, accompanied by one of the parents or other persons authorized by one of the parents with a notarized consent (application)

Documents

To certify the authenticity of the signature on the application with the consent of one of the parents, the following documents are submitted to the notary: the passport and identification number of the taxpayer (other) of the father/mother, the child's birth certificate.

The application indicates the country of destination and the period of the child's stay in this country.

When parental permission is not required

There are certain cases when consent is not required when crossing the border:

1) if the second parent is a foreigner or a stateless person and this is confirmed by an entry about the father in the child's birth certificate, and who is (not) absent at the checkpoint or if in the passport of a citizen of Ukraine for travel abroad, with which the citizen crosses the state border, who has not reached the age of 16, or the child's travel document is a record of departure to a permanent place of residence outside Ukraine or a note of registration in the consular register at a diplomatic mission or consular institution of Ukraine abroad.

2) in case of presentation of documents or their notarized copies, namely: death certificate of the second parent; court decision on deprivation of parental rights of the second; the court's decision to recognize the second parent as missing; the court's decision to recognize the second parent as incompetent; a court decision on granting permission to leave Ukraine to a citizen who has not reached the age of 16, without the consent and accompaniment of the second parent; birth certificate of the child, issued by the civil status registration department, indicating the grounds for entering information about the father in accordance with the first part of Article 135 of the Family Code of Ukraine (when the child travels abroad accompanied by a single mother).
